Registration & Waiver Form (PDF) - This form is required before any training can begin at Competitive EDGE Athletics. Click on the link to open the form in a new window. Please print and complete BOTH pages of this form and bring it with you to the gym. BOTH pages of this form must be signed by either a parent or legal guardian of the student/athlete. If you have already completed this form through the online registration process, you do not need to complete it again.
Absence/Vacation Request Form (PDF) - This form Legacy Extreme cheerleaders/dancers to request an absence or vacation.
2009 - 2010 Legacy Sponsorship Form (PDF) - This form is for 2009 - 2010 (NewSeason) Legacy All-Star sponsorships.
Referral Form (PDF) - Use this form to get cash credits for Legacy Extreme, CEA Tumbling, and ZUMBA class referrals. This form must be completed and turned in by the person being referred before Legacy Extreme or CEA tumbling registration or ZUMBA punch card purchase.
Booster Funds Transfer Request (DOC)- Use this form to request that funds be transferred out of your booster account for qualified expenses.
